Guests who have an active OP (operational processing) number issued by WestJet’s Medical Desk can request travel using this form. Please be aware of the following:

Your OP number, typically OP plus 3-5 digits (E.g., OP12345), is required for this booking. The team cannot access your accommodation details without this number. If you do not have your OP number, you will have to email MedDesk@WestJet.com to request this information.
Your OP number must be active on your intended date of travel. If it is not, you cannot use this form to request travel.
If you do not provide all of the information required in the form below, we cannot complete your request.

If your OP number has expired and needs to be renewed, please submit the appropriate form for your travel needs to MedDesk@WestJet.com. To request an attendant or an extra adjacent seat, complete a new Air Travel Requirements Assessment form. To request a seating accommodation, complete the Accessible Seating Accommodations form.

- Eating meals, taking medication, using the toilet, transferring to/from a seat, orientation or communication, physical assistance in the event of an emergency, evacuation or decompression.
