Online request for a seating accommodation

Guests who have an active OP (operational processing) number issued by WestJet’s Medical Desk can request travel using this form. Please be aware of the following:

  • Your OP number, typically OP plus 4-5 digits (E.g., OP12345), is required for this booking. The team cannot access your accommodation details without this number. If you do not have your OP number, you will have to contact the Medical Desk to request this information.
  • Your OP number must be active on your intended date of travel. If it is not, you cannot use this form to request travel. 
  • If you do not provide all of the information required in the form below, we cannot complete your request.

If your OP number needs to be renewed, please fill out a new medical information form and email meddesk@westjet.com.

- Capable of and needed to provide assistance to a person with a disability (after departure/before arrival) with:

- Eating meals, taking medication, using the toilet, transferring to/from a seat, orientation or communication, physical assistance in the event of an emergency, evacuation or decompression.

- Not be responsible for the needs of any other guest on that flight, including an infant or child.

- Remain with, and be seated adjacent to, the guest with a disability (Medical Desk documents any exceptions in the STAR).

- Be fully capable of attending to their own and the guest's physical and mental care needs during the flight.
